Summary

The purpose of this program is to implement Assisted Outpatient Treatment (AOT) program in communities to support adults with serious mental illness (SMI) who meet state-specific criteria for AOT. Recipients are expected to implement a multi-disciplinary AOT program, working with courts, community partners, and other entities to support community-based treatment for adults with SMI who meet criteria for AOT.

The AOT grant program is one of Substance Abuse Mental Health Service Administration's services grant programs. SAMHSA intends that its services grants result in the delivery of services as soon as possible after award.
AOT program components should include:
• Evaluation of the psychiatric and social needs of participants;
• Development and implementation of treatment plans related to the medical, psychiatric and social needs, including the criteria for court-ordered treatment;
• Monitoring participant compliance with the treatment plan;
• Providing case management services that support the treatment plan;
• Making referrals to medical and social service providers for additional services;
• Evaluating the program implementation process to ensure consistency with participant needs, preferences, and state law; and
• Measuring treatment outcomes, including health and social outcomes such as rates of incarceration, health care utilization, and homelessness.
Eligibility Details

Eligibility is statutorily limited to counties, cities, mental health systems (including mental health authorities), mental health courts, or any other entity with authority under the law of the state in which the applicant grantee is located to implement, monitor, and oversee AOT programs.

Full application details are anticipated to be posted in the late spring 2025.
Award Details

Anticipated total available funding is $15,229,744. The anticipated number of awards is up to 30 awards with up to $750,000 per year. Length of Project is up to 4 years. No cost sharing/match is required. Anticipated Project Start Date- July 31, 2024.
Annual continuation awards will depend on the availability of funds, grantee progress in meeting project goals and objectives, timely submission of required data and reports, and compliance with all terms and conditions of award.
